Epigénétique, microenvironnement et cancer du foie (M. Levrero/M. Zeisel)
Le carcinome hépatocellulaire (CHC), le principal cancer primitif du foie, représente une cause majeure de transplantation hépatique et de décès par cancer dans le monde. La grande majorité des cas de CHC peut être associée à un facteur de risque sous-jacent bien caractérisé, tel qu’une infection chronique par les virus de l’hépatite B, D et C (VHB, VHD, VHC), la consommation excessive d’alcool ou encore des troubles métaboliques (maladie hépatique stéatosique associée à un dysfonctionnement métabolique (MASH)). Bien que le risque de développer un CHC puisse être réduit chez les patients par le traitement de la cause sous-jacente (p. ex. par l’élimination du VHC, la suppression de la réplication du VHB, ou l’abstinence), il n’y a pas de stratégie efficace pour prévenir le développement du CHC chez les patients atteints de fibrose avancée et de cirrhose. Le CHC avancé est de très mauvais pronostic et les options thérapeutiques restent largement insatisfaisantes.
L’inflammation chronique, les dommages à l’ADN et les modifications épigénétiques contribuent à la transformation des hépatocytes ainsi qu’au développement et à la progression du CHC. Tous les facteurs étiologiques semblent agir par le biais de mécanismes similaires qui convergent pour affecter des voies communes. Au cours des dernières années, notre équipe a caractérisé i) les changements épigénétiques qui précèdent et accompagnent le développement et la progression du CHC; ii) l’interaction du VHB, du VHD et du VHC avec l’épigénome de l’hôte; iii) les bases moléculaires et immunologiques de la pathogénie virale et de la persistance virale dans le contexte des maladies hépatiques chroniques liées aux VHB et VHD.
Projets de recherche
Dans le but d’étudier les changements épigénétiques dans les CHC viraux et non viraux, nous nous intéressons plus particulièrement aux
1) histones méthyltransférases en tant que « driver » épigénétiques du CHC et cibles thérapeutiques
2) protéines du VHB et du VHD comme modulateurs épigénétiques dans la pathogénie virale et le développement du CHC
De plus, nos axes de recherche translationnelle incluent la mise au point d’un test diagnostique pour accompagner le développement des traitements curatifs de l’hépatite B ainsi que de nouvelles stratégies pour améliorer la qualité des greffons pour la transplantation hépatique. L’équipe fait également partie d’un réseau européen pour évaluer de nouvelles stratégies immunomodulatrices pour guérir l’hépatite B.
Nos projets de recherche bénéficient d’un vaste réseau de collaborations nationales et internationales permettant l’accès à des cohortes de patients VHB/VHD, VHC et CHC et de la participation directe de membres de l’équipe EpiLiCan au programme de recherche clinique de l’Hôpital de la Croix Rousse.

Projet PROFIL NASH

Le but de cette étude est la caractérisation du microenvironnement inflammatoire intrahépatique chez les patients atteints de stéatohépatite métabolique par transcriptomique, immunophénotypage et protéomique fonctionnelle.
La maladie stéatohépatique est la 1ère cause de maladie dans le monde, favorisée par nos modes de vie actuels. Elle peut parfois entraîner des complications graves, comme la cirrhose et le cancer du foie, notamment chez les patients avec inflammation intrahépatique marquée.
Aucun traitement n’a démontré à ce jour une efficacité pour ralentir la progression de la maladie, hormis la perte de poids. De plus, l’identification des patients à risque de progression nécessite actuellement une ponction-biopsie hépatique. Une technique innovante micro-invasive, l’aspiratoire fine à l’aiguille (FNA), permet d’isoler en grande quantité des cellules inflammatoires du foie.
Cette étude permettra de développer d’une part des marqueurs non invasifs (tests sanguins) pour identifier les patients à risque de progression (avec inflammation intrahépatique marquée et fibrose) et d’autre part permettre d’améliorer nos connaissances dans la pathogénie de la maladie en étudiant les cellules inflammatoires intrahépatique.
Cette étude porte sur les patients atteints de stéatopathie métabolique et indication de ponction-biopsie hépatique, suivis dans le service clinique d’Hépatologie et Gastroentérologie de l’Hôpital de la Croix-Rousse, au sein des Hospices Civils de Lyon. Quarante patients sont attendus dans l’étude. La durée de participation à la recherche est limitée au jour de la biopsie de foie, où seront réalisés les prélèvements sanguins et la FNA.

Le but de cette étude est de déterminer la composition du microbiote intrahépatique, sanguin et fécal à J0 chez les patients suspects d’hépatite alcoolique sévère.
L’hépatite alcoolique (HA) est une complication grave de la maladie alcoolique du foie (MAF). Sa présentation histologique est caractérisée par des foyers de nécrose hépatocytaire associés à une infiltration de polynucléaires neutrophiles (PNN), une ballonisation hépatocytaire et la présence de corps de Mallory. En cas d’HA sévère, définie par un score de Maddrey supérieur ou égal à 32, la mortalité à 1 mois est estimée entre 10 et 50%. La seule thérapeutique permettant une réduction de la mortalité précoce est la corticothérapie. Toutefois, seul 60% des patients répondent à la corticothérapie et aucun bénéfice n’a été démontré sur la mortalité tardive. L’identification de nouvelles cibles thérapeutiques constitue donc un enjeu majeur dans cette pathologie. La composition des différents microbiotes sera déterminée par le séquençage du gène de l’ARN ribosomal (ARNr) 16S, et les résultats seront corrélés aux données cliniques (corticosensibilité, survie globale, survie sans transplantation, score de MELD notamment) et histologiques.
La composition du microbiote intrahépatique (obtenu à partir de la biopsie hépatique), sanguin et fécal sera analysée chez les patients suspects d’HA sévère suivis dans les services d’Hépatologie et Gastroentérologie du Pr. ZOULIM, d’Hépato-gastroentérologie du Dr Guillet, et d’Anesthésie-Réanimation du Pr AUBRUN à l’Hôpital de la Croix-Rousse, au sein des Hospices Civils de Lyon. Cinquante patients consécutifs avec suspicion clinique d’HA et indication de PBH transjugulaire seront inclus. La durée de participation à la recherche est de 6 mois, où seront réalisés les prélèvements sanguins, fécaux et la ponction-biopsie hépatique à la 1ère visite et à 6 mois. Les données cliniques, biologiques usuelles et l’évolution clinique du patient seront consignées à J0, J7, M1, M3 et M6.
