SPOTLIGHT
By integrating RNA-seq and multiplex immunofluorescence on paired samples from the NeoPembrOv phase II trial, our work revealed key determinants of response to neoadjuvant chemotherapy plus pembrolizumab in high-grade ovarian carcinoma.
- Combination therapy increased intraepithelial CD8⁺PD-1⁺ T cells
- A composite endothelial-monocyte-T cell signature predicted response with AUC = 0.93
- High CD8B/FOXP3 and CD8B/ENTPD1 ratios correlated with response, while VEGFR2 expression marked resistance
These findings highlight regulatory T cells and VEGFR2⁺ endothelial cells as potential targets to overcome immune resistance in ovarian cancer.
